Every Middle School or Upper School student planning to participate in the CFS Athletics Program during the 2008 - 2009 school year must have a 2008-09 CFS Permission and Emergency form as well as a current sports physical form completed and signed by their physician before practices start.  This form can be downloaded by clicking the link below.  It is also available from Nancy in the Center.
